As we continue to grow, we're looking for professionals who can be the steady force behind every case, advocating, coordinating services, and guiding individuals and families. Key Responsibilities: • Maintain regular contact with patients and their families to ensure a high standard of care • Complete all required monthly and annual documentation accurately and on time • Coordinate patient transportation and medical supply orders • Conduct quarterly in-person visits to assess patient needs and build relationships • De-escalate situations when patients are dissatisfied • Maintain regular office hours, Monday–Friday, 9:00 AM–5:00 PM. Ideal Qualifications: • Associate's or Bachelor's degree required • Strong organizational, communication, and problem-solving skills • Proficient in basic computer skills (email, documentation, data entry) • Experience in case management or patient coordination is a plus • Bilingual ability is a plus Salary: Starting at $52k–60k/Year.
